Add new field to customized dialog

Hi,

I am playing around of provisioning dialog customization. I copied miq_provision_amazon_dialogs_template dialog and changed an existing fields and description. Via my domain I assigned this customized dialog to users and this part works. But I didn’t find a way how to add new field to the customized dialog.

The fields in the provisioning dialogs are not dynamically generated which means the views also need to be updated if you want to expose completely new fields.

Take a look at this PR where two new fields where added: :resource_group and :root_username
